The Department of Health and Human Services (DHHS) shall charge the following registration fees:
- For an initial application (Qualifying Patient and Designated Caregiver); $50
- For a renewal application (Qualifying Patient and Designated Caregiver): $50
- For issuance of a replacement registry identification card: $25,
(Except when a designated caregiver is issued a new card due to a change in his qualifying patient’s registry identification number, there shall be no fee charged)
All fees are non-refundable and shall be made payable to “Treasurer – State of New Hampshire”.
